T.E.A.M Funds Application

Team Recovery is proud to introduce the T.E.A.M. Funds, a dedicated initiative to support agencies in Lucas County working tirelessly against addiction. Our mission is to foster unity among community resources and amplify our collective efforts against addiction’s challenges. By offering this funding, we aim to strengthen our communal solidarity and magnify our shared values.

Required Documentation

Upon submitting your application, you will also be required to submit the following documentation, so please have it ready:

• supporting documentation demonstrating you primarily serve adults/adolescents with mental health or addiction issues.
• proof of accreditation from OMHAS and either CARF or JCAHO
• please email Amy Priest at “apriest@lcmhrsb.oh.gov” and cc “info@theteamrecovery.org” to request approval for Team Recovery’s “Team Funds”
• a budget breakdown. Funds should be used within 60 days.
